What is the survey about?
This survey is about your experiences at school (or your child’s experience) since being diagnosed with diabetes. We want to understand what is working well and what could be better.
What will happen to my answers?
The National Paediatric Diabetes Audit (NPDA) team at the Royal College of Paediatrics and Child Health (RCPCH) will analyse the responses and create:
- A local report for your diabetes team to help them plan improvements to their service
- A national report to support improvements in diabetes care across wider areas
Who should complete the survey?
- Children and young people who have diabetes and are attending primary school, secondary school, college, or are home schooled/educated.
- Parents/carers of children and young people of all ages who have diabetes and are in nursery, primary school, secondary school, college, or are home schooled/educated. If you have multiple children with diabetes in education, please complete one response per child.
Is the survey anonymous?
Yes
We do not ask for names or details which could identify you, so your local diabetes clinic or
school will not know which answers are yours. If you are completing the survey in clinic, you
should have the opportunity to do so in private.
If you would like to enter the prize draw for a £50 voucher, we will ask you to enter your email address.
Your email address will not be linked to your survey responses and will be deleted as soon as a winner is selected.
Completing the survey?
The survey should take 10-15 minutes to complete.
To complete the survey, select the answer(s) that best match your experience, or use the free text boxes to type your response where they appear.
There are no right or wrong answers, please just tell us what you think and chose what you think is the most appropriate answer to the question.
When you have completed your answer, select the ‘NEXT’ button to move to the next page.
If you change your mind, you can click the ‘PREV’ button at the bottom of the page to return to the previous page.
Your participation is entirely voluntary. You do not have to take part if you do not want to. You can exit the survey at any point by closing your web browser.If you do not wish to answer some of the questions, you do not have to give a reason.
